ABSTRACT Mining and metals have long carried one of the highest fatality rates of any industrial sector. In the decade leading to 2015, major operators achieved meaningful safety advances— separating injury and fatality prevention strategies, strengthening critical risk management, and investing in organisational culture and operating models. These changes delivered benefits as they matured after 2015, and several leading companies later demonstrated that sustained fatality-free performance was possible through multi-year “zero fatality” runs. Yet those records were subsequently disrupted, and industry-wide fatality totals (including among ICMM members) have again risen, signalling a deterioration after a period of proven success. Drawing on 15+ years of corporate HSE leadership and intervention experience across 45–50 surface and underground operations globally, the authors identify a recurring systemic syndrome behind this drift. First, “system complexity inflation” has occurred: organisations continuously add new safety tools and requirements, under the guise of continuous improvement, without retiring legacy ones, increasing volume, ambiguity, and bureaucratic burden. This encourages superficial compliance rather than authentic risk control. Second, workforce demographics and contracting models have shifted markedly—loss of tacit knowledge, higher turnover, expanded outsourcing, and language/literacy barriers—creating a widening mismatch between frontline needs and the accessibility of corporate systems.
