In that environment, safety professionals often became politically constrained. To demonstrate value, they improved and expanded the existing architecture—adding sophistication, tools, and requirements—rather than doing the harder work of simplification, retirement of legacy artefacts, and redesign around frontline realities. Decommissioning older systems can be polarising, particularly when those systems were installed by current senior leaders. Introducing “innovations” can appear more progressive than removing clutter, even when removal would have greater impact. At the enterprise level, implicit trade-offs likely occurred: falling behind competitors on ESG and carbon metrics may have been perceived as a higher commercial risk than pursuing further step-changes in fatality elimination. Even if not formally evaluated, these judgments may have underestimated the full economic value of effective safety—through reliability, productivity, cost control, and avoidance of disruption. The result was an acceptance of status quo fatality frequencies and the conditions for gradual deterioration. The operating environment is now shifting again. Investors have become more sophisticated in evaluating sustainability performance and in differentiating genuine valuecreating investments, such as automation and artificial intelligence, from superficial activity. Expectations are returning to operational fundamentals—execution quality, productivity, and margin improvement. This creates an opportunity: rebuilding the next generation of efficient operations, can be paired with a renewed fatality elimination strategy, where fatality prevention is both a catalyst for operational excellence and a major dividend of success. CONCLUSIONS AND IMPLICATIONS FOR INDUSTRY Companies—particularly those that have experienced fatalities or serious injuries in the last five years—should consider a “fatality prevention re-set.” This should be a fundamental review of the collective approach to safety and fatality prevention, grounded in the realities of the current asset portfolio and today’s workforce demographics, capabilities, and contracting models. Given the geographic and operational diversity of many mining groups, companies should also reconsider whether strict global standardisation inadvertently ignores local workforce needs and operating conditions. A more tailored local approach, aligned to enterprise intent but designed for local usability, may deliver better outcomes. A fatality prevention re-set could include the following actions: 1. Test for presence of the syndrome across operations (complexity inflation, tool overlap, ambiguity, compliance-driven behaviours, and declining control quality).
